Service Rates

Listed here are the list of all of our service rates and fee policies, including late cancellations & missed appointments. All clients are required to have a card or form of payment on file before our initial session. This card payment will be apart of the clients online account and can be updated via their client portal. Participation in counseling services is an agreement to pay all associated fees, unless canceled within a 48hr period.

Out-of-Network Policy: Superbill

  • A superbill shows an itemized list of services that you have received from an out-of-network provider. It's like a detailed invoice or receipt. A therapist may provide you with a superbill — a specific document required by an insurance company — that allows you to seek reimbursement for an out-of-network provider- GoodRx Health

  • Required Information for superbill:

  1. Client contact information​

  2. Provider information

  3. Your diagnosis

  4. CPT code

  5. Dates of service

  6. Itemized list of cost

  • Superbills will be sent to the clients portal account via simplepractice.​

  • Additionally, a quote of benefits is not a verification and insurance reimbursements are based on actual information and claims submitted and is subject to eligibility, terms, limitations, and exclusions of your health care program.

  • you (not your insurance company) are responsible for full payment.

Late Cancellation & Missed Appointment Policy

  • All cancellations should be completed within a 48hr period. Failure to cancel via phone call, email, or client portal notification will be acknowledge as a no-show session resulting in a full session charge of $150.00. Any late cancellations not made within the 48hr period will occur a $100.00 late fee. 

  • Multiple Insufficient funds will be penalized by a $ 35.00 dollar additional fee upon next sessions payment charge.

  • Participation in services is an agreement to pay all associated fees of that provided service. All clients are required to have a form of payment on file for processing.
